Original Joe Frazier fight application from 1966. Three page application first being white, the second both pink, with two leaflets stapled to front page. One a serology report, the other labeled “RETURN TO PHYSICIAN” Both stamped by “Dr. Waxman” dated 5/19/66. The top of the application reads “REPORT OF PHYSICAL EXAMINATION OF APPLICANT FOR LICENSE” with “PROFESSIONAL BOXER” field checked off. The bottom is marked “Unsatisfactory physical condition to be licensed as a Prof. Boxer.” The document is signed at the bottom by Frazier in black ink rating an 8. The next two are Eye Examination Reports of Frazier at ages 22, 23. Both are signed in blue ink by Frazier the first rating a 6, and the latter a strong 7. Both eye exams report Joe to be 20/20 in both eyes. When interviewed about fighting Ali in “The Thrilla In Manila” Joe was quoted saying “"My left eye went when I was young. I was working the speed bag and some steel went in the eye and scratched it to pieces. I was kinda blind in that eye. "Some doctors were my best friends. We sheltered the story and kept it a secret. And I learnt the eye-chart by heart." This means that Frazier basically could not see out of either eye in his epic victory over Ali in Manila. The reports and application are in EXMT condition overall. The lot is accompanied by an LOA from Frazier’s agent, as well as a JSA LOA.
