On the back inside collar is the “Starter Center Ice” manufacturers tag. Below is the “56R-8-21” strip tag. On the front is the large "Avalanche" logo patch embroidered in burgundy, blue, silver, and white on white tackle twill. On the back and on both sleeves is the player number "22" in white on blue on silver triple tackle twill. Across the back is the player name "FORSBERG" in white tackle twill on a burgundy name plate. On each shoulder above the number is the "Avalanche" logo patch embroidered in burgundy, blue, and silver on white. On the lower right back is the “STARTER” and "NHL" embroidered logos. On the inside back is the original fight strap. On the front left breast is the Alternate Captains "A" patch in white on blue on silver triple tackle twill. Autographed on the back number in black marker and rates an 8. The jersey shows game-use and appears to have no alterations of any kind. Accompanied by a LOA from the Colorado Avalanche and Milt Byron. The autograph is accompanied by a LOA from JSA.
